$125 per 50-minute session

Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully.  Your mental health coverage contact phone number is typically on the back of an insurance card.  You may also call my billing office and they will contact your insurer to determine what your benefits are.  The billing office phone number is 414.321.6458.  If you call your insurer, be sure to ask these questions:

• Do I have mental health benefits?
• What is my deductible and has it been met?
• How many sessions per calendar year does my plan cover?
• How much does my plan cover for an out-of-network provider?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician?

Reduced Fee
Reduced fee services are available on a limited basis.
